Submitting the Monthly Report

Step-by-step guide for completing and submitting the LIA monthly report form.

LIA supervisors are required to submit a monthly report documenting program activity for their MIECHV zone. Reports must be submitted through the Supervisor Portal at defamilysupporthub.org and are due by the 10th of the following month.

Step-by-Step Submission

  1. Log in to defamilysupporthub.org using your supervisor credentials.
  2. Go to your zone dashboard. After logging in, you will land on your zone's main dashboard view.
  3. Click "Submit Monthly Report." This opens the monthly report form for the current reporting period.
  4. Complete all required fields:
    • Reporting month and year
    • FSS caseload counts (active families by FSS staff member)
    • Visit counts by home visiting model (HFD, NFP, PAT, or EHS)
    • Training completions for the reporting period
    • Any program notes or narrative updates
  5. Review your entries before submitting. All required fields must be filled in.
  6. Click Submit. You will see a confirmation message on screen immediately after submission.
  7. Check your email. A confirmation email will be sent to your address on file.

Missed Deadline

Reports are due by the 10th of the following month. If you are unable to meet the deadline, contact Emily Thompson at DPH before the deadline passes. Late submissions may affect compliance standing.
